Your care journey

What happens after you start?

01

Share your concern

Tell us what is bothering you, how long you have had it, and what you think we should know.

02

Bridge review

Your submitted information can be reviewed and organized so the next stage has a clearer picture of your case.

03

Case preparation

Relevant symptoms, history and available records are brought together for the next step in your care journey.

04

Doctor care

Your case moves toward suitable doctor care and consultation when the relevant stage is ready.

Start with your concern, not a diagnosis.

The first step is simply telling us what is bothering you.

Why this matters

Healthcare can be difficult before the appointment even begins.

The patient experience starts with understanding what you need, not with expecting you to know the healthcare system already.

Not sure which doctor to start with?

You can begin with your concern instead of trying to diagnose yourself or choose a specialty first.

Have reports or prescriptions?

The intake flow lets you provide relevant files and additional context as part of your case.

Want your information organized?

Your submitted symptoms, history and available records can be structured for the next stage of your journey.

The patient journey

From your first message to your next care step.

The exact timing and next action can depend on your case. The important part is that your information moves through a defined workflow instead of leaving you to figure out every step alone.

The Bridge step

Your case should be easier to understand before the next conversation.

The current Upchar360 workflow includes a Bridge review stage. Your submitted information can be reviewed and organized before the case moves toward matching and doctor care.

Your main concern

What is bothering you and what you want help with.

Symptoms and duration

How long it has been happening and how severe it feels.

Previous reports

Relevant prescriptions, lab reports or scans you have available.

Additional context

Previous diagnoses, medications, allergies or anything important to mention.

Use the walkthrough to understand the product flow before you begin. It is a product demonstration, not medical advice or a diagnosis.

The product flow shown in the video may change as the platform evolves. Your actual case can have different next steps depending on its status.

Common questions

Before you begin.

A few things patients commonly want to understand about the journey.

Do I need to know which specialist I need?

No. You can start by explaining what is bothering you. The patient journey is designed to help you move toward the appropriate next step without requiring you to diagnose yourself first.

What information do I provide?

The current patient intake asks for your main medical concern, contact phone number, how long you have had the issue, symptom severity, relevant files such as reports or prescriptions, and additional context such as previous diagnoses, medications or allergies.

What happens after I submit my information?

A ticket is created for your case. The current workflow then moves through Bridge review, case preparation, matching, doctor assignment and consultation as the relevant stages are completed.

Can I track my case?

Yes. The patient dashboard provides active tickets and their current progress, along with access to ticket details.

Is Upchar360 a replacement for a doctor?

No. Upchar360 is a healthcare access and coordination platform. It does not replace professional medical diagnosis or treatment.

How is my information handled?

Review the current Privacy Policy and the platform's security controls before submitting sensitive health information.
